The purpose of this study was to assess the level of maxillary sinus floor in dentulous, edentulous and free end cases among Malay population based on gender, and age by using panoramic radiographs. Sample size was 364 patients consisting of 118 dentulous, 192 edentulous, and 54 free end cases. Measuring the volume of maxillary sinus and distance between maxillary sinus floor and root apices of upper posterior teeth at both sides was performed in dentulous cases. Measuring the volume of maxillary sinus and distance between the maxillary sinus floor and residual ridge at all molar regions was done in edentulous and free end cases for both sides. Regarding the study of the distance and volume in relation to gender and age, results showed that the distance in dentulous showed no differences between both sides, also age and gender presented no effect on the distance in dentulous patients. The distance for each edentulous and free end showed no differences between left and right sides. While the relation of this distance with gender noted differences for each edentulous and free end cases with the reduction in distance in males more than females. The relation of the distance with age showed decreasing in the distance with increasing age for each edentulous and free end cases. The measurements of maxillary sinus volume presented different volumes at both sides in dentulous cases, while it wasn't different in edentulous and free end cases. The relation of the volume with gender showed no differences volume in dentulous, edentulous and free end cases. Finally it was found that the relation of the volume with age was not affected by increasing age in dentulous and free end cases, while it showed no relations with increasing age in edentulous cases.
