520 |a Objective: This study was designed to evaluate the radiological outcome and recurrence of patient with benign cystic bone lesion treated operatively with synthetic bone substitute. Methods: Fourteen cases of benign cystic bone lesions treated surgically in Hospital Tengku Ampuan Afzan from January 2006 till June 2015. All 14 cases were treated with the same technique, excision and extended curettage, flushing and saline irrigation, followed by calcium sulphate injection. Radiological healing is measured according to Chang scoring system at interval of 6, 12, 24 and 48 months. Result: Inter observer bias was validated in scoring system by Chang with Kappa analysis reported as (0.109, 0.340, 0.462 and 1.000 respectively) at follow up interval. Initial assessment show low agreement 0.109 and increased to moderate agreement in 2 consecutive interval (0.340-0,462) with excellent agreement at final interval of follow up (1.000). Patients with early healing accounts for a total number of 8 (57%) achieved early healing at 6-12 weeks and 5 (36%) at 24-48 weeks post procedures. Mean average healing time was 14.1 weeks. At 6 weeks postoperatively almost half of the total number of cases achieved early healing with complete healing at 24 weeks postoperatively. 1 (7%) case continued to have persistent cyst and 1 (7%) had recurrence after complete response at 24 weeks. There is no significant difference in bone healing and recurrence within these groups (p>0.01). The other parameters which we compare includes sites of lesion within upper limb and lower limb, size of lesion and amount of graft used which made no difference in the outcome. Conclusion: Benign bone lesion treated with synthetic bone graft have a very good response rate and low morbidity comparable with autologous bone graft.
