Summary:Excellence students in PMR and SPM examination results in the school relies heavily on collaboration between students, teachers, administrators and parents. In order to improve the school's academic performance is highly related to the teacher's role in producing excellent students. The improvement of education, testing, measurement and evaluation is very closely related to each other because it can help teachers and students learn the knowledge, understanding and mastery of a subject that has been presented. Assessment is normally carried out in the form of tests, exams, theory, exercises, quizzes, practical and so on. The purpose of this study is to determine whether the practice of teacher assessment results can influence the improvement of student achievement. Researchers have modified Azhar Testing Instruments 2007 in identifying the level of usage of evaluation of teachers in schools with the concept of evaluation and measurement with high reliability through pilot with Alpha value 0.837 is analyzed. Data collected through questionnaires were analyzed using descriptive (percentages and frequencies) are using t - test, and the correlation (inference). T-test was used to compare the scores of students in achievement based on sex between men and women, while the correlation analysis carried out to study the relationship between the practice of measurement and evaluation carried out by the use of outcome measurement and evaluation, student achievement and student opinion examinations for teachers in improving student performance on exams. The results show excellent frequency counts and percentages of female students was higher than male students. Correlation results showed that there was a strong correlation between the level of utilization of the practice teachers share the learning objectives, the use of oral questions, provide feedback and encourage self and peer assessment.
